Do I need to fill these out if I'm an existing patient?
Usually, no. These forms are for new patient intakes. However, if your medical history or insurance has changed significantly, we may ask you to update them.
Can I email the forms to you?
For your security, we recommend not emailing forms that contain sensitive data (like SSN) unless you use an encrypted service. It is safer to bring the physical copies or use our secure Patient Portal.
